Your First Visit
Please arrive about 10 minutes early so that you have enough time to fill out a New Patient Form. For added convenience, you can fill out the digital New Patient form below.
Initial Examination
Your dentist will start by reviewing your dental and general medical history, as well as by conducting a comprehensive examination of your smile. This exam will include a checkup of your bite, jaw, gums and teeth as well as oral cancer screening and any required X-rays.
Treatment Plan
Based on your history and examination, your dentist will develop a long-term dental treatment plan according to your needs. We will also take time to answer any questions you might have about your dental health and your treatment options.
Insurance Claims
We can direct bill your insurance company, and our administrative team is happy to assist you in understanding your insurance plan better.
Payment Methods
The full treatment cost must be paid at the time of your appointment. We accept payment by cash, debit or most major credit cards.

To cancel or reschedule an appointment, please call us. Please inform us about any changes to your plans as soon as you can, so we can offer the space in our schedule to another patient and reschedule you as quickly as we can.
- Do you offer appointment reminders?
In the days before your appointment, you will receive automated phone or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
- What languages other than English do you provide services in?
Yes, our office is able to provide dental services in several languages including French, Hindi and Punjabi.
